Pay Policies
For the convenience of employees, their pay is automatically deposited in their personal bank or credit union accounts by noon on payday. Employees can distribute their pay up to four different ways, guaranteeing they receive their money on time every time.

Variable Compensation Plans
DLO's Goalsharing Plan recognizes and rewards employees for the continuous improvement of their organization. This plan is paid in cash. All regular full-time, part-time and on-call employees with more than six months of service are eligible to participate. Eligible earnings do not begin to accrue until after six months of service.

Health & Dental Insurance
DLO offers the option of a Consumer-Directed Health Plan through either Lumenos or Aetna Choice POS II Medical Plan from Aetna. The company pays more than 80% of the premium for these plans.
Those DLO employees who are regularly scheduled to work at least 30 hours per week are eligible to participate in one of the company's medical or dental plans. Eligible dependents include qualified domestic partners who can enroll in the Lumenos or Aetna Choice POS II medical plans. Elected coverage takes effect the first of the month coincident with or next, following one month of employment.
Dental PPO coverage is a stand-alone plan and may be purchased without carrying medical insurance.

Pre-Tax Savings Plan (the 401(k) Plan)
After completing one month of service, DLO employees may
invest up to 35% of their pre-tax income in any one, or a combination of over
15 available, investment options with Fidelity Investments. The available
options include investment funds, lifecycle funds, and a company stock fund.
After 12 months of service, the company matches employees' contributions,
dollar for dollar, up to 6%. This match is non-forfeitable, i.e., fully vested,
at all times.

Employee Stock Purchase Plan (ESPP)
DLO employees may purchase Quest Diagnostics Incorporated stock each calendar quarter, at a cost of 85% of the market price at either the beginning or end of that quarter, whichever is lower. From each paycheck, a minimum of $10 and a maximum of 10% of the employee's pay may be invested. Through a quarterly enrollment, The ESPP is available to employees regularly scheduled to work at least 20 hours per week. Dividends paid on purchased stock are automatically reinvested.

Time Off with Pay (TOP)
DLO employees are provided Time Off with Pay. TOP may be used for vacation, personal needs, and incidental sick time. Hours are accrued each pay period based on the hours worked.
Employees begin accruing TOP after three months of service. Full-time employees (scheduled to work 40 hours per week) accrue 5.54 hours per pay period, or about 13.5 days for the remainder of the first year. At the first anniversary of employment, the accrual increases to 6.15 hours per pay period, or approximately 20 days per year. The rate of accrual continues to increase with longer service.
At the end of an employee's first calendar year, all TOP is carried over for use in the next year. At the end of each subsequent year, the ability to carry over and be paid unused TOP is subject to statutory requirements. To be eligible for this benefit, the employee must be regularly scheduled to work 20 or more hours a week.
All accrued but unused TOP is paid to employees when they leave the company.

Scheduled Holidays
In addition to TOP, those employees who are regularly scheduled to work 20 or more hours per week are also paid for six holidays each year: New Year's Day, Memorial Day, Independence Day, Labor Day, Thanksgiving, and Christmas Day.

Short-Term Disability (STD)
The company provides employees with disability coverage through Aetna at no cost to the employee. Effective after three months of employment, all employees regularly scheduled to work at least 30 hours per week receive this benefit. It becomes payable on the eighth day of absence due to non-work related injury or illness.
Short-term disability benefits pay eligible non-exempt employees up to 60% of their weekly pay for up to 26 weeks. The plan also continues 100% salary for eligible exempt employees for up to 26 weeks.

Long-Term Disability (LTD)
In case of disabilities continuing after 26 weeks, DLO also provides each eligible employee with long-term disability insurance. This benefit is paid on a monthly basis as long as the employee is disabled, up to age 65 (later if the disability begins after age 60). Payments equal 60% of the employee's monthly pre-disability earning, up to $15,000 per month. Effective after three months of employment, this benefit is provided to all employees regularly scheduled to work at least 30 hours per week.

Educational Assistance
After six months of employment, DLO reimburses tuition paid by employees regularly scheduled to work at least 30 hours per week for pre-approved, job/career related courses taken at an accredited institution. Full reimbursement of eligible expenses (tuition) is provided for two courses per semester. Reimbursement is subject to a "B" grade or above.

Flexible Spending Account (FSA)
Employees use an FSA to set aside (through pre-tax payroll deductions) up to $5,000 per account for expenses related to eligible health care or dependent/child care. Employees regularly scheduled to work at least 30 hours per week are eligible to participate in these accounts after one month of employment.

Life Insurance
DLO pays for base life insurance, AD&D, and business travel accident insurance coverage for eligible employees at no cost to the employee. The amount of base life insurance equals 3 times the annual base pay for Exempt employees and 11/2 times the annual base pay for Non-Exempt employees. Employees may purchase supplemental life and AD&D insurance up to 2 times their annual base pay. Coverage is effective after one month of employment for all employees regularly scheduled to work at least 30 hours per week.
