Note regarding Aetna:  If you are a Primary Care Physician and wish to have DLO serve your Aetna HMO patients, please contact your Aetna Provider Relations Representative and request an HMO Laboratory Designation Form. You must first submit your lab change form to Aetna, before using DLO. Specialist Physicians may use the lab without first contacting Aetna. If you have any questions, please contact your Aetna Representative.
