Methodology: Immunoassay (IA)

Clinical Significance: Vancomycin is an antibiotic that is bactericidal against many gram-positive and some gram-negative cocci. It is useful in therapy of severe staphylococcal (including methicillin-resistant staphylococci) infections and in patients who cannot receive or who have failed to respond to the penicillins and cephalosporins.

Due to the bi-exponential decay of vancomycin concentration over time, the drug does not lend itself to traditional pharmacokinetic approaches. It is imperative that the drug is infused at the same rate and time and that the peak is drawn at the same time for subsequent comparisons to be valid. Toxic concentrations can used nephrotoxicity especially when administered with another aminoglycoside. Especially when administered with another aminoglycoside.

Collection Guide: 

Supply: T05 - Red Top Tube (NO GEL) 10mL (x2 for Vancomycin, Total)

Preferred Specimen: Serum;
Vancomycin, Peak - Collect the peak specimen 1-2 hours after completion of a 60 minute IV infusion

Vancomycin, Trough - Collect the trough specimen just before administering next dose

Vancomycin, Total - Collect the Peak specimen 1-2 hours after completion of a 60 minute IV infusion and Collect the Trough specimen just before administering next dose.

Preferred Volume: 2mL (per tube for Vancomycin, Total)

Transport Container: Red Top Tube (NO GEL)

Transport Temperature: Room Temperature

Specimen Stability: Room Temperature: 2 days

Reject due: Specimens collected in SST (serum seporator tubes).

Special Instructions: CANNOT be run from SST tube. MUST be collected in a red top tube NO GEL; For the Vancomycin, Total, 2 specimens are required, 1 peak specimen and 1 trough specimen.
