In order to receive your DLO Direct™ test results, you will need to create a MyQuest™ account. Since lab test results are your private health information, you must use the exact same name and demographic information you included on the DLO Direct order form in order to view your secure results. Your lab results will be linked to your account based on the information you provide on the order form, so your MyQuest account creation information must match exactly to properly view your lab test results.
Follow the simple steps below to create your account or download a document of the steps.
Go to myquest.questdiagnostics.com and click the green “Sign Up” button.
If you have not previously created a MyQuest account, click the green “Register Now” button.
Fill in your personal information and click “Submit.” In order to receive your results, you must use the exact name and demographic information you included on the DLO Direct order form. Your results will be linked to your account based on the information you provide. (Example: If you wrote "Michael Smith" on the DLO Direct order form and use "Mike Smith" when creating your MyQuest account, the data will not link. It must be exactly the same.)
Create a username and password. Your password is case sensitive, must be at least 8 characters and include at least one alphabetic and one numeric or special character. Select two security questions from the drop-down list and click “Submit.”
Review the Quest Terms and Conditions. If you agree, click the box next to the “I have read and accept the Terms and Conditions” statement. Review your security and personal information for accuracy. If you need to make a change, click “Edit.” If your information is correct, click “Submit.”
Quest will send you an email to confirm your account. Click the “Account Confirmation page” link in the email to confirm your account.
Now that your account has been created, you must verify your identity. Login to MyQuest by clicking the black “Login” button and enter your username and password.
Click the green “Verify Your Identity” button near the top of the page.
Review the Patient Authorization language. If you agree, click the box next to the “I have read and accept the Terms and Conditions” statement and click the “Authorize” button.
Enter the last four digits of your social security number and click “Continue.”
Answer the security questions to verify your identity and click “Submit.” These questions will be unique to you and will be generated based on information Quest receives from the personal information you have provided.
Your account has now been completed and you will be able to your results through MyQuest in 7-10 business days following your visit to a DLO Patient Service Center. You will receive an email when your results are available to be viewed.
DLO Direct uses the MyQuest portal through our partnership with Quest Diagnostics to bring you your lab results. Since MyQuest is utilized by Quest Diagnostics customers nationwide, there are some functions in the portal, such as making a lab appointment that DLO does not offer. Please only use the MyQuest portal to access your lab results.
