    PATIENTS AND PROVIDERS: DLO's Portland Ave. Patient Service Center, located at 5401 N.     Please note: To receive lab work, you'll need a lab order from your healthcare provider, or an order purchased through QuestHealth.com.


    Preparing for your Patient Service Center (PSC) visit, Lab Results and Billing:

    Appointments and Wait Times 
    Appointments are not required but are prioritized over walk-in visits. To schedule an appointment, visit the webpage your preferred Patient Service Center location and select the “Make an Appointment” option. 
    Please note that Patient Service Centers are typically busiest during the early morning hours.

    Lab Results
    Test results are sent directly to your healthcare provider and made available through MyQuest™, Quest Diagnostics’ online patient portal provided by DLO. 
    To learn more about MyQuest™ or create an account, visit the link above. If you would like a personal copy of your lab results, please contact DLO Customer Service at 1-800-891-2917 (Option 2) to request a Request for Access form. You may also contact your healthcare provider directly for results. 
    Most routine test results are available within 24 hours; however, some specialized tests may require several days or weeks for processing.

    Billing and Insurance
    The cost of laboratory testing may vary depending on factors such as insurance coverage, patient age, and contractual agreements between your healthcare provider and DLO.

    DLO files insurance claims with contracted insurance providers designated as in-network. Claims submitted to non-contracted insurance plans may be processed as out-of-network, which could result in patient responsibility for payment.

    Preparing for Your Lab Services

    Laboratory testing procedures are typically quick and straightforward. Depending on the test ordered, you may need to fast or temporarily avoid certain medications before your appointment. Please follow any instructions provided by your healthcare provider prior to your visit.

    Blood Testing
    Before your appointment, confirm whether any dietary or medication restrictions apply to your test. 
    During the visit, a healthcare professional will collect a blood sample from your arm. You may feel a brief pinch or sting during the collection process. Afterward, pressure will be applied to the collection site. 
    Your specimen will be analyzed using specialized laboratory equipment, and results will be sent to both your healthcare provider and MyQuest™.

    Urinalysis
    For urine testing, a healthcare professional will provide a sterile specimen container along with collection instructions. Once collected, the sample will be labeled and sent to the laboratory for analysis. 
    Results will be made available to your healthcare provider and through MyQuest™.

    * COVID-19 PCR testing at a DLO Patient Service Center is for asymptomatic patients or patients exhibiting mild, cold-like symptoms. Patients who are exhibiting active flu-like symptoms, including fever, may be asked to reschedule their appointment.
