Urinalysis Tube Supply Change *Effective 10/13/2025

This notification is to inform you that effective October 13, 2025, the U03 Urinalysis Tube will be discontinued and replaced with U73 Urinalysis Tube with yellow top (199896).

To order the new U73 Urinalysis Tube, please download the Supply Request Form and email the completed form to dloclientsupply@questdiagnostics.com (see above).

T-Spot Supply Change

DLO recently changed how you can order T-SPOT testing supplies (does not include shippers).

This notification is to inform you that effective November 18, 2024, T-SPOT.TB® supplies (tubes, bags, and absorbent material) have been consolidated to item number K145 and can be ordered through Quanum, email DLOClientSupply@questdiagnostics.com, or call 1-800-891-2917. Supplies will now be fulfilled through DLO’s Supply center.

PLEASE NOTE:
T-SPOT kit contains 5 Lithium Heparin tubes, 5 white bags, and 5 absorbent sheets. Clients, please use order code K145 and indicate the number of kits that are needed. For example, 20 kits would be required for 100 samples.

How do I order T-SPOT shippers?
To order T-SPOT shippers, email TSPOT@QuestDiagnostics.com or call 1.877.598.2522 to reach T-SPOT client services directly. Shippers will continue to be monitored by par levels and replenished as needed.
